Problem
Check the published panel, roles, channel, and challenge configuration.
Start at Advanced Moderation > Member Verification.
Common causes
- The panel snapshot is stale or missing.
- Verified or Unverified roles are above the bot role.
- The verification channel or delivery permission is unavailable.
- The challenge expired, reached its attempt limit, or is in cooldown.

Inspect the panel publication status and selected channel.

Compare role hierarchy and Manage Roles.

Check challenge expiry, attempts, cooldown, and member age requirements.

Review the configured success/error response and verification history when available.
Fix

Repair roles and channel permissions.

Save and update or republish the panel.

Wait for the configured cooldown or issue a new short-lived challenge.
